Anthem Blue Cross And Blue Shield Of Indiana To Implement Pay-For-Performance Program  Anthem Blue Cross and Blue Shield of Indiana is planning to increase payments by as much as 10% for physicians who follow recommended care guidelines, the Indianapolis Star reports. Anthem, health care facilities at odds :At least 10 health care providers to opt out of new contract.  At least 10 Indiana health care facilities -- including one Michigan City hospital -- plan to opt out of the Anthem Blue Cross and Blue Shield network in November, according to Joe Stuteville, the media relations manager for Sisters of St. Francis Health Services Inc.
