News 2942 Humana Medication Therapy Management Medicare

In accordance with the Medicare Modernization Act of 2003, Humana has released its new Medication Therapy Management (MTM) program. This program is available to Humana members who have more than one chronic disease such as diabetes, have anticipated costs of medications above $4,000 for the 2006 calendar year and take more than seven Medicare Part D prescription medications within a 90-day period. These Humana members will receive a monthly drug statement in the mail called SmartSummary Rx (SM). This statement will list their individual monthly prescription medication expenses and provides information designed to help each member get the most out of their prescription drug benefits. Humana will also work with their network of pharmacies to help participating members understand the medicines they are taking and utilize a fact-to-face outreach approach.
